Thank you for submitting your Missionary Cooperation Appeal Application to the Catholic Foundation for the Diocese of Green Bay. Please know that we notify all applicants of their status around the first week of February.
Please read the following instructions carefully.
1. You will receive an email from the Mission Office at the address you entered in the form thanking you for completing your application and providing additional instructions. Attached to this email will be a PDF copy of your application.
2. You will need to mail any required documents you did not attach to your application to the Catholic Foundation. Applications will not be processed until all documents are received. The deadline for submitting the application is November 1.
3. Mail required documents not attached to the application to the Catholic Foundation – World Missions Office, PO Box 22128 Green Bay, WI 54305-2128
