Please read the following instructions carefully.
1. Please see the PDF copy of your application, attached to this email.
2. You will need to mail any required documents you did not attach to your application to the Catholic Foundation – World Missions Office, PO Box 22128, Green Bay, WI 54305-2128. Applications will not be processed until all documents are received. The deadline for submitting the application and any required documents is June 15.
